Translation

Θεοτόκε βοήθι Εὐ(φη/θυ)μίᾳ μοναχῇ.

Theotokos, help the nun Euthymia/Euphemia.

Commentary

The Ν in the last line has the appearance of an Η; still the reading of it as μοναχῇ is most likely.
