Translation

Κύριε βοήθει τῷ σῷ δούλῳ Κωνσταντίνῳ ταπεινῷ μοναχῷ.

Lord, help your servant, the humble monk Constantine.

Commentary

The reading of the name Constantine and the monastic epithet ταπεινὀς is tentative, but there is little doubt that the first letters of the third line on the reverse are ΚΩ.
