Translation

Ἀναστ / θανασίῳ διακόνο τῆς Μεγάλης Ἐκλισίας καὶ κουβουκλεισίῳ.

Anastasios / Athanasios deacon of the Great Church and kouboukleisios.

Commentary

Either Athanasios or Anastasios is possible based on what remains of the second line. However, we would argue that the latter is perhaps more likely, as the lower portions of two letters visible on the first line seem to be NA.
