Κύριε βοήθει τῷ σῷ δούλῳ Θεο... διακόνῳ βασιλικῷ κληρικῷ καὶ πατριαρχικῷ νοταρίῳ.

Lord, help your servant Theo... deacon, imperial cleric, and patriarchal notarios.

Commentary

The owner's name will likely have been Theodore, although Theodoulos or even Theopemptos are also possible.
