Inscription in four lines with decorations above and below. Border of dots.

Translation

Κύριε βοήθει τῷ σῷ δούλῳ Ἀντωνίο μοναχῷ.

Note the which takes up two vertical lines, as opposed to the Χ being superscripted above the Α.
