Translation

Θεοτόκε βοήθει Ἀνδρέᾳ μοναχῷ κουβουκλεισίῳ καὶ βασιλικῷ (?) νοταρίῳ.

Theotokos, help Andrew, monk, kouboukleisios and imperial (?) notarios.

Commentary

Because of the missing half seal, it is unclear what (if any) word precedes the office of notarios; in this period it is most likely "imperial," though protonotarios is also possible.
