Translation

Ἅγιε Νικόλαε βοήθει τῷ σῷ δούλῳ Ἰωάννῃ βασιλικῷ κλιρικῷ καὶ κουβουκλεισίῳ.

St. Nicholas, help your servant John imperial cleric and kouboukleisios.

Commentary

The first two letters of the second line are not entirely clear; however, they seem to read Ν, a · marking an abbreviation, then a , thus making this John an "imperial" cleric. The other less likely reading is that they are ΝΗ, and thus that he is a non-imperial cleric,
