Translation

Κύριε βοήθει τῷ σῷ δούλῳ Θεοδώρῳ μοναχῷ τῷ Ἀφραζιάτῃ.

Lord, help your servant the monk Theodore Aphraziates.

Commentary

The end of the second line of the reverse is effaced. There is enough room for two letters, in which we posit the letters ΑΧ, the standard abbreviation for μοναχ, or monk.
