Translation
Θεοτόκε βοήθει τῇ σῇ δούλῃ Εὐδοκίᾳ μοναχῇ καὶ δούλῃ τῆς Τριάδος.
Mother of God, help your servant Eudokia, nun and servant of the Trinity.
